If your student will be taking daily medication during school hours, please stop by my desk at Meet the Teacher.
All doctor's excuse notes need to be turned in to me when the student returns to school.
We do not keep over-the-counter medication in the school clinic.
All medication, prescribed and over the counter, requires a medical permission form. This form must be filled out and signed by the student's doctor.
